Application for long-term care benefits
Here you will find the application for long-term care benefits.
You need this if you want to use long-term care insurance benefits for the first time – i.e. primarily to apply for a care level. After the application, the long-term care insurance fund usually commissions an assessment (e.g. by the Medical Service). On this basis, the long-term care insurance fund decides on your care level and what benefits you are entitled to. We recommend that you make use of long-term care advice in accordance with §7a SGB XI before submitting your application.

Application for Respite Care 2026
Here you can download the application for substitute care.
This is how you notify your long-term care insurance fund that you would like to use the budget for substitute care. After activation, you can use substitute care if your private support person (caregiver) is temporarily unavailable or needs relief. Fill out the application and send it to your long-term care insurance fund.

This document helps you quickly clarify what budget is still available to you in 2026.
You can use it to specifically ask your long-term care insurance company which amounts have already been used, which budgets are still open, and whether there are any accumulated remaining amounts (e.g., from the relief amount). The long-term care insurance company can give you concrete information on this – so you reliably know what you can plan with before you book support.

Application for severe disability
Here you will find the application for the determination of severe disability.
With this, you apply to the responsible office for an assessment of your degree of disability (GdB) and – if the requirements are met – a severely disabled person's ID card.
The procedure is not handled by the long-term care insurance fund, but by the State Office for Health and Social Affairs (Lageso).

The application for a care level is often crucial for the start: it triggers the assessment and determines which services are fundamentally possible. Respite care is a separate budget for relief when a private caregiver is absent or needs a break. A budget query helps you clarify the current status of your funds with the care insurance in writing so that you can plan reliably. If you clearly separate the differences, you avoid typical mistakes: "wrong application", "wrong fund" or missing evidence.
How do you meaningfully query your care budget 2026 – so that you get a concrete number (instead of general statements)?
Formulate the request so that the long-term care insurance fund has to name amounts: What budgets are there, how much has already been used in the current year, how much is left, and whether there are transferable/accumulated amounts (e.g., for the relief amount).
